| Please note that the group coordinator must complete the Birthday Party Agreement form and mail the completed form with the nonrefundable deposit to Historical Happenings in order to reserve the date. A Disclosure of Risk form must be completed for each individual participant and submitted on the day of the event. Children who do not have the completed form will NOT be allowed to participate. |
